Most people are probably going to say something along the lines of Grand Theft Auto already being a video game, but there are many other expansions on the video game genre that you could do with this game rather than just making a "Grand Theft Auto 4, the second". I want to create a racing game, that takes into account all the updated physics and car styles in the newest Grand Theft Auto and add them to the ability that Gran Turismo gives you in customizing your cars. You have to win missions, and buy cars (or steal them). When you get cash you start upgrading the pieces to your car, like new shocks or a turbo kit.
You will start the game with no car, and no money. There will be someone there to teach you how to steal a car, and the better the car the harder it will be to steal. Through stealing cars your theft skill will go up and you can get better and better cars. So you could decide to start racing with the first car you get, or you could keep stealing cars and try and get a really good car to start with, either way works. Once you have completed the initial car theft and decide to start racing you will have to just drive around until you manage to find people actually racing through the streets, and chace after them. Once you beet them enough times through different races they will pull you over and help you familiarise yourself with their world.
Once you have entered the world of the racers you can start getting calls about racing different people at different places, build rivalries and even get in with the underground race shops. When you get in to this zone you will be able to customize all the parts of your car and even calibrate them as you want. Want racing shocks? Fine, but are they set tight or loose, how will you tune your turbo kit. What parts of your car exactly do you want to remove when you are reducing the weight.. All of these things cost money, and money is only earned through racing. It isn't a system where you will just be able to win a race and get cash, you have to put money up or your car up as a bet. If you loose, you loose the car or the cash.
The game is never really over, you will keep getting new rivals and the further in the game you get the better the rivals will be. Eventually they will take corners perfectly and drive really well. Maybe you can keep up and maybe you can't, this wont be a game where you play it for a week and destroy every computer opponent there is.
